We team up with the Connecticut Department of Social Services (DSS), Community Options (COU), the Department of Mental Health and Addiction Services (DMHAS), Connecticut Community Care (CCC/CCCI), the Area on Aging organizations in Southwestern and Western Connecticut (SWCAA and WCAAA), Allied Community Resources (ACR), Access Health, and United Services. Our partnerships also include working with well-known local universities and hospitals like UCONN, Yale, and Hartford to make sure we're offering the best care possible. As a dedicated provider of community care and supported living, our goal is to offer top-notch, personalized support that really makes a difference in people's lives. Whether it's through the Medicaid Money Follows the Person (MFP) program, the ABI Waiver Program, or the PCA Waiver, we're here to ensure everyone we help gets the support they need in a way that respects their individuality and strives to improve their everyday life.
