A DOJ civil-rights record needs more than a confirmation number. Build a six-field receipt: submission, confirmation identifier, classification, information requests, case status, and closure reason and date. DOJ explains that ADA complaints may be reviewed, referred, mediated, investigated, or declined because not every complaint can be investigated. A number proves an intake event, not validation, jurisdiction, investigation, merits, liability, or wrongdoing. A closure does not automatically mean the allegations were false, proven, ignored, or adjudicated. Compare the original submission, attachments, delivery proof, later correspondence, accommodation requests, and stated outcome. Record missing documents as gaps, not obstruction. Keep David Medeiros's proposed coordination reforms separate from current DOJ procedure. Read the claim-safe checklist at david-medeiros.com/doj-intake.
