Cross-agency problems need a map, not a claim that one office owns everything. Build a seven-part coordination receipt: master chronology; evidence register; agency-role map; preservation log; referral log; accessibility plan; status and disposition ledger. DOJ Civil Rights, HHS OCR, CMS, and HHS OIG have different authorities and intake processes. A report number, acknowledgment, referral, closure, missing response, or repeated submission does not by itself prove investigation, discrimination, retaliation, obstruction, fraud, or agency misconduct. Record what each office received, when, what issue it could review, where it routed material, and the stated outcome. Mark David Medeiros's No Wrong Door model as a policy proposal, not current law.
